H&SS Outstanding Service Award Nomination Form

1. Job Performance:

Individuals display excellence in job performance by producing quality work consistently and dependably. They display a deep knowledge or an expertise in their field. Exhibiting pride in accomplishment, they strive toward job-related self-improvement and seek to enhance both product standards and work efficiency.

2. Dedication:

Dedicated individuals accept responsibility and make the extra effort to complete a task. They are willing to put in extra time and try alternative methods as necessary. They are determined problem-solvers who are not easily discouraged by challenging situations.

3. Positive Attitude:

Individuals display courtesy, helpfulness, self-assurance and enthusiasm. They are strong communicators who do not focus on the obstacles at hand, but rather on overcoming those obstacles.

4. Citizenship:

Individuals have a sense of community within their department, the college, and the university. They have strong interpersonal skills, and show respect for the ideas and feelings of others. They share information and offer their time to others. They build harmonious working relationships, based on loyalty and commitment, that improve productivity and promote shared goals.
